Service Description: Assist families to enhance their child or youth participation in their community and support the family to advocate for services that they require. Work in partnership with agencies, organizations and families to foster an inclusive continuum of services and facilitate the community's capacity to respond to the family's needs.

Coordinated Service Planning supports the development and monitoring of a service plan for individuals with multiple and/or complex needs up to 18 years of age and youth between 18 and 21 years if still in school. Service navigation and facilitation of team meetings are also provided.

FASD Coordinator supports the development and monitoring of a service plan for individuals with FASD or suspected FASD up to 18 years of age and youth between 18 and 21 years if still in school. Service navigation, capacity building and facilitation of team meetings are also provided.

Coordination Services for Children and Youth facilitates service planning for a child or youth up to 18 years of age, when the complexity of the child's needs are beyond the capacity of the existing system. Works in collaboration with families and community service providers.

Family Networking - Hosts several family events throughout the year such as the annual family day held in September, the children's Christmas party in December and support groups held on an on-going basis.

Early Learning Inclusion staff support inclusion and capacity building in the early learnind and care community. Enhanced Staffing - Provides additional staffing in licensed child care centres and after school programs to facilitate the inclusion of children with high needs.

Resource Library - An extensive library of books, articles, periodicals and videos pertaining to special needs and is available to both clients and the community.

Dual Diagnosis Respite Funding - For children between 8 to 18 years of age with a mental health and developmental disability.

Transition Activation Worker - Funds to hire a worker to support transition back to community from hopital or residential treatment.
