Service Description:
|
Provides in-home nursing and personal support care services * services can be short-term or long-term and may include wound care, IV therapy, injections, and other nursing services * services provided in client homes, long-term care homes, retirement homes, rehabilitation centers and hospitals and include: - senior care - light housekeeping and meal preparation, mobility, bathing and dressing
- in-hospital care - support and companionship during hospital stays
- post-op and disease recovery care - change dressings, administer medication, monitor for complications, assist with personal care, accompany to follow-up appointments, help with basic household tasks, run errands)
- rehabilitation care - help with personal care and daily activities and offer support with physical therapy and exercise programs
- mental healthcare - for patients of any age living with mental health challenges including crisis stabilization, respite care, suicide watch and psychogeriatric care including dementia and Alzheimer’s disease
- alzheimer’s and dementia care – specialized in-home support to improve safety, comfort, and quality of life
- companionship and family support – providing social engagement and emotional well-being for individuals and their families
- palliative care - for individuals with life-limiting conditions * includes escorted transportation to medical appointments, therapy sessions, and errands
