Service Description: Ontario Health atHome provides in-home and information and referral services by care coordinators in the Lakeridge Health Corporation Port Perry Hospital.

The role of the hospital care coordinator is to assess and arrange for required Ontario Health atHome in-home care services upon discharge from hospital and to link patients to other useful community resources.

In-home services may include personal care, nursing, physiotherapy, occupational therapy, speech language therapy, dietetic services and social work services.

Fees: Covered by OHIP
Application: Although initial inquiry may come from patient/family member, the formal referral must come through a hospital professional such as a physician, social worker or nurse.
Waiting Period - Case management services are available prior to discharge from hospital
Eligibility / Target Population: Must be a resident of Ontario with a valid Ontario Health Insurance Plan (OHIP) card. Must be a patient in the Port Perry Hospital. Further eligibility will be assessed upon initial intake according to the Long Term Care Act 1994 (see attached).
