Service Description:

Helps clients remain in their own homes for as long as safely possible while deferring the need for hospital admission or long-term care * services are offered to seniors who reside at home, but need the availability of additional unscheduled visits * services include:
- personal care (dressing, bathing, personal hygiene, medication assistance, meal preparation)
- security checks
- light housekeeping
- care coordination

Eligibility / Target Population:

High Risk Seniors who are 65 years of age and older * must reside in the designated geographic areas where the service is provided.
