Service Description:
|
Help to aging adults and adults with special needs to settle at home in a timely manner after a hospital stay
Services include:- transportation home
- picking up medications and groceries
- light meal preparation
- personal care and homemaking services provided by a Community Care Personal Support Worker
- follow-up telephone calls to check on the well-being of the client and when necessary, linking the client with additional community-based services
