Health centre focused on primary care and family and community health * health professionals assess and treat non-life-threatening injuries or illnesses * diabetes education * geriatric assessment * community development/health/promotion and education * referrals to other local health services and personal development groups * priority populations are youth and older adults
A range of community-based health services are provided by an inter-professional team including physicians, nurse practitioners, registered nurses, registered dietitians, health promoter, community health workers, social worker, and other health professionals
