Service Description:
|
Hospital-based outpatient program for older adults typically aged 65 and older who live at home or in a retirement residentce who have complex medical, functional, and pychosocial problems conditions, have recently experienced functional or cognitve decline, or have experienced falls or at high risk for falling. Services are provided by an interprofessioanl team that includes nurse practitioners, behavioural support nurse, occupational therapist, pharmacist, physiotherapist, Home, Community Care Support Services Central East Care Coordinators, and geriatrician or physician who specializes in geriatrics available for consultation when requrired..
Older adults receive an assessment and are treated, as needed. Recommendations and treatment plans are created and shared with the older adult and primary care provider.
