Service Description:

Helps seniors and their caregivers navigate the health care system ensuring they are connected to the care and services that are needed and wanted * support when seniors are faced with life changes and significant care needs, including:- referrals to community resources and programs
- client advocacy
- crisis intervention
- help with long-term care options
Education and information sessions include:- wellness sessions
- counselling and tours
